These mistakes rarely exist in isolation. They often collide in practice. Take “vibe coding,” where engineers build AI tools with no guardrails, no transparency, and no alignment with business or workforce planning. The result is shadow AI projects, security gaps, unclear accountability, and customer confusion. That is not innovation. That is risk multiplied.

So how do you avoid these traps? Start with intention. Redesign processes before deploying models. Modernize your data. Train your teams. Establish guardrails. Create governance that includes every function, not just IT. And always keep humans in the loop for decisions that require judgment, empathy, or nuance.
